Two Lives Lost During 3-Car Accident in Carroll County

34-year-old Madonna Woodward (Berryville, AR) and an unidentified 16-year-old male minor were both fatally injured during an accident that took place on Monday, November 19.

At approximately 12:00 p.m., Ms. Woodward was driving a 2002 GMC Envoy east on Highway 62 just east of Green Forest in Carroll County. Ms. Woodward’s vehicle crossed the center double yellow line and struck a westbound 2002 Chevrolet Tahoe, before rotating and striking a 2006 Honda Pilot.

The driver of the Honda Pilot was the unidentified 16-year-old minor who was fatally injured in the accident. 66-year-old Hector Calederon-Moreno (Green Forest, AR) was a passenger in the vehicle.

Two passengers, 34-year-old Rigoberto Tovar-Tellez (Berryville, AR) and an unidentified 7-year-old female were in the Chevrolet Tahoe.

Those injured were transported to North Arkansas Hospital in Harrison.

Weather conditions at the time of the accident were cloudy and road conditions clear.
